Abstract
Based on North Sumatra health profile data in 2017, the number of maternal deaths was recorded at 205 deaths, lower than the data recorded in 2016 which was 239 deaths. The highest number of maternal deaths in 2017 was recorded in Labuhan Batu Regency and Deli Serdang Regency with 15 deaths, followed by Langkat Regency with 13 deaths and Batu Bara Regency with 11 deaths. The purpose of this study is to provide continuous midwifery care Continuity Of Care to pregnant women TM III (34-36, maternity, postpartum, and family planning by using a midwifery management approach with the SOAP method. This research is a type of research used is descriptive with a study approach The subject in this study was Mrs. S with Continuity Of Care, GIP0A0 from pregnancy to family planning acceptor. After a case study of continuous care (COC) was carried out on Mrs. F from pregnancy to postpartum and family planning and BBL care, where Mrs. F's pregnancy was a physiological pregnancy, Mrs. S's delivery was spontaneous vaginally. Ny. S's involution postpartum period was normal, Mrs. S's baby was normal, there was weight gain according to age, there was no infection in the umbilical cord and the baby was breastfeeding regularly. exclusive, post-partum family planning, Mrs. S and her husband chose MAL, it is known that continuous care is effective in preventing pregnancy loss both morbidity and mortality. It is recommended for health workers to continue to provide continuity of care for mothers and to improve the health and welfare of mothers and children and to reduce maternal and child mortality.
